How Referrals and Home Visits Work

Clear, responsive coordination for clients, families, nurse case managers, physicians, adjusters, and attorneys.

1. Referral & Coverage Verification

A physician, case manager, attorney, adjuster, or family member contacts us. We verify Auto No-Fault or Workers' Compensation coverage and authorization before scheduling.

2. In-Home Evaluation & Plan of Care

A licensed NeuroHaus occupational therapist completes a thorough, unhurried evaluation in the client's home. Together we set measurable goals, and those goals are shared with the care team.

3. Home & Community Treatment

One-on-one sessions focus on real routines: daily living skills, cognitive strategies, adaptive equipment, and Letters of Medical Necessity when needed. Progress updates go to the care team throughout.
